    Request for reimbursement accompanied by documentation must be made within 90 days of the completion date.

    Dollar amounts for tuition, fees and books should be as accurate as possible.
    Final reimbursement request total cannot exceed the amounts listed below.
    Do not include a $ in your entries. You will get an error!
